Do Doctor's King "hallucinations," comeback at tier 2 after snapping out?

So the Doctor has addons that make certain things happen at madness 2-3 such as the King which does it each time they are shocked. My question is if you get all 5 effects on someone and then they go back to madness 1, when they go back up to tier 2 do all five effects come back or only one at a time again?
