Pallet confusion

Please explain to me how pallets work. If the screen says hit R1 to heal why will survivor reach offscreen or grab a pallet 5 meters away and drop it, but if you are standing in front of a pallet and it says hit R1 to drop it the player will begin to self heal. How does the game determine what function to assign to R1?
