Iron will & surveillance
I think that surveillance and the new ruin are a combo that guarantees you two kills each game
with iron will the entire point of the perk is to not scream yet when the doctor shocks you you scream? I get that doctor needs a way to find you but iron will should still work

Iron Will's purpose is to prevent you from moaning
Calm Spirit prevents you from screaming
Can't say I agree about Surveillance and Ruin. Once the totem goes, that combo breaks fast. And trying to defend the totem means you have zero gen defense.
