On 'frustration' as a motivator for changing things

The patch notes indicate that the motivation for the upcoming changes to doctor and ruin are to do with frustrations survivors have when playing against them. I'm curious as to why this isn't a measure used more consistently. If frustration is really the reason why these changes are being implemented, there are several things way more frustrating to survivors than doc and ruin.

Noed for example is one of the most hated, rage inducing killer perks that even killer mains avoid out of sportsmanship. You have no warning that it's present until you or your team-mate is insta-downed and your endgame is ruined.

Ebony Mori's are and always have been frustrating. Completely unfun to verse and typically elicits DCs.

Huntress' iri head (typically combined with infantry belt) is universally recognised as the most OP add-on in the game. Highly frustrating when you're on one effective health-state for the entire game. Especially at no added risk to the huntress.

And for killer, its extremely frustrating to verse survivors who know how to loop, do gens too quickly, or use comms to eliminate your killer advantages.

Down really, really low on the list of what frustrates survivors is arguably doctor. If the notes are to be believed, then why aren't the serious frustrations being addressed at all? Why start from the bottom of the list?

As for ruin, I honestly don't think there are any devs who understand the game meta. Why is ruin used in 80% of red rank games? Is it because its powerful? if that were true then noed would be meta. but it's not. It's actually gen speed. There is a disparity in how long it takes to complete all the gens vs the expected time to kill 2-3 survivors. At red ranks, this is the most apparent.
